Please note: You WILL NOT receive a dental ID card from MetLife. You only need to inform your dentist that you are enrolled in the MetLife Preferred Dentist Program (group number is 0215113) and they can easily verify your enrollment information by contacting MetLife.
If you wish to cancel your current coverage of dental insurance, please complete the cancellation request form and return it to the Human Resources Department. Form is available here. Please note: You must be enrolled in the Dental Plan for one full year before you may cancel it.
